This page will contain the forms and documents you need to complete your incorporation process. You must download all of the documents you see listed on this page. Some browsers will automatically download some of the documents, and you may not see it happen. If you do not see the document open up in your browser window, you can check your "downloads" folder, or whatever folder you usually download items to, to see if they are there.
Documents with file names ending in '.pdf ', can be printed blank, and filled out by hand, or can be filled out on your computer, then printed and signed. It is your choice which way you fill out these forms.
To successfully use the PDF documents you will need to have the Adobe Acrobat Reader DC program installed on your computer. You can download this program for free from this link. Use the link 'Download now", and NOT the one for the "free trial". When you install this program, one of the steps should ask you if you want to set the reader DC as your default reader for PDF documents. You will want to say YES, so that when you save the documents to your hard drive they will respond correctly. Speaking of which, most of this cannot be done on a phone, or even some tablets, unless you have all of the apps you need for documents. Use of a computer or laptop is recommended.
The Bylaw Templates and group inclusion letters have been created in Microsoft WORD, and are done so in order that you can fill in your particular chapter or state information without re-typing the entire document. If you do not have Microsoft Word on your computer, the document probably will not respond properly. Please follow all instructions included in the Start-Up OR petition kit.

Please open and save all documents, and read all instruction documents, before filling out the corresponding forms.
